This is a multi-part assembly designed to be printed in four colors and glued together. Assembly is a little tricky as there are several small pieces. I recommend a pair of needle nosed pliers for the smallest.
If you are fortunate enough to have a 4-color printer you can probably print it in one go. I don't, so please leave a comment if you do and are able to use it successfully.

Rafting is not strictly necessary unless your printer has problems with small pieces. Because there are many small pieces, I find it convenient to print with a raft to keep them together.
My recommendation for gluing is Gorilla brand Super Glue Gel. The Gel is specifically handy because it does not run and is more forgiving.
I started with the grey backing, then glued in the larger black pieces. Then the white and blue pieces one at a time. And finally the small black pieces.
I started with a base flat image (see below) which I cleaned up into consistent solid colors. I then used a program I wrote to separate out each color into a mesh, with magenta as the background and grey as a base. To the blue and white meshes I applied a 100% -> 200% Z scaling along the Y axis, to give the effect of the wings rising from the plaque.
